The Calgary Police Service and MedicAlert Foundation Canada are working together to improve how first responders get information about anyone who may require special attention in an emergency due to a medical, mental health or physical condition, or due to a cognitive developmental disability.
Disability tax credit certificate
Individuals who have a severe and prolonged (defined on the next page) impairment in physical or mental functions, or their legal representative, use this form to apply for the disability tax credit (DTC) by completing Part A of the form.
